At Rockport Family Dental, we love new patients and would like to invite you into one of our comfortable, clean and beautiful operatories, overlooking lush flowers and immaculately groomed landscaping! Take your mind off of your busy day and enjoy a thorough cleaning and comprehensive exam tailored custom to you and your smile. We look forward to hearing from you and scheduling your first visit. In order to make your first visit as smooth as possible simply print the forms, complete, and bring with you to your first visit. Or, you can send them to us beforehand (via email, fax, or mail). Once reviewed, we can discuss your needs and answer any questions you might have.
For each additional family member, please fill out separate forms. If you are in need of urgent or emergency dental care, please call the office at 978-546-3020 during normal business hours before filling out a form or send an email to rockportdental@gmail.com and our dentist will respond as soon as possible.
During printing/completing our New-Patient forms if you have any questions please don’t hesitate to chat with a Treatment Coordinator by using our chat box on the lower right corner of our website and we will help you as quick as possible.
Your First Visit
Your first dental appointment at Rockport Family Dental will be about getting acquainted with each other. First, we will discuss your goals and what ultimately brought you to our practice, review and explain treatment options and learn about your medical history. After, a clinical evaluation will be performed and all questions answered.
Some routine dental services, if time permits, can be performed the same day as the consultation. In certain circumstance such as more complex services, or even if you have issues in your medical history, we will schedule a second appointment with you for treatment. At that appointment, we’ll thoroughly review our findings and present a detailed dental treatment plan that addresses your dental health needs and esthetic goals.
Ways to make your first visit a breeze:
- X-Rays. You can either request your previous doctor send them to us before your appointment date or bring them with you, however, in many circumstances new X-rays will be indicated.
- Completed patient registration forms listed below. This will save you time at your appointment and allow us to spend more time discussing treatment options.
- List of any medical prescriptions or conditions that may impact your treatment.
- Please bring your dental insurance card and any necessary completed forms, including referral paperwork (if applicable). This will also save time and allow us to help you process any claims.

Patients under 18 must be accompanied by a parent or guardian on their first consultation visit.
